Force-Feed Esme or Refuse

Browse Choices Guides
Quick answer

Helping Pieter or refusing changes the opening scene. Both branches lead to getting herbs from Anca; the later medicine recipe is the important survival decision.

Recommended
Choose either response for role-play. The opening force-feed choice changes the scene's tone but has no confirmed long-term gameplay consequence.
Quest impact
Esme's survival depends on obtaining and brewing the correct medicine before the prologue deadline, not on this opening response.

PC Gamer reports the same immediate objective for both choices. The linked video shows their opening scenes; it does not establish every possible long-term effect on the current patch.

What actually affects Esme's survival?

The later Withering Away medicine step is separate. PowerPyx specifies hot water and three spoonfuls of herbs, completed before time runs out. Follow the in-game recipe and save before preparing it.

What can you compare in the video?

The original comparison is by RoamWithRebble, uploaded September 3, 2026. Selected frames were checked directly on October 8:

Video positionWhat is visibleWhat it supports
00:05Pieter holding Esme and asking Coen to try againThe helping scene
01:06Pieter directing Coen to Anca for stronger herbsThe next objective after helping
01:18Coen refusing to helpThe refusal scene

These checks use the subtitles visible in the gameplay, not an exported automatic transcript. The shared next objective for both branches is also reported by PC Gamer. The video does not verify Esme's later survival or a current-patch ending; its build is unknown.

Your next step

Continue to Anca, get the herbs, and return for the medicine objective. If you are also considering Page-Turner, compare its extra segment with your remaining prologue time.
